Do I need a referral?
Yes, a referral is required by state law to initiate treatment by a licensed physical therapist.
The state of Indiana requires a referral for both evaluation and treatment of a patient.
How do I get a referral?
Referral to Physical Therapy should be in writing and signed by the primary provider.
A referral can come from a family physician, an internal medicine physician, an OB/ GYN, a surgeon, a Physician's Assistant, a Nurse Practitioner
and any other medical specialist.
What should I expect on my first visit?
During your first visit you can expect the following:
How should I dress?
- - Arrive at your appointment 10-15 minutes early with your paperwork completed or to fill out your paperwork.
- - You will provide us with your referral for physical therapy.
- - We will copy your insurance card.
- - You will be seen for the initial evaluation by a licenses physical therapist.
- - The therapist will discuss the following:
- 1. Your medical history.
- 2. Your current problems/complaints.
- 3. Pain intensity, what aggravates and eases the problem.
- 4. How this is impacting your daily activities or your functional limitations.
- 5. Your goals with physical therapy.
- 6. Medications, tests, and procedures related to your health.
Loose fitting clothing so you can expose the area we will be examining and treating.
What should I bring on my visit?
Make sure you bring your physical therapy referral (provided to you by your physician)
and your payment information. If your insurance is covering the cost of physical therapy,
bring your insurance card. If you are covered by Workers' Compensation, bring your claim number and your case manager's
contact information. If you are covered by auto insurance or an attorney lien, make sure you bring this information.
What does a Physical Therapist do?
We are experts at evaluation and treatment of musculoskeletal dysfunction. We are specifically trained to improve your mobility, strength,
function and relieve pain. Physical Therapists help a person recover from an injury, surgery and aid in the recovery from a disease process
or illness. TMC Rehab uses joint and soft tissue mobilization techniques, exercise, heat, ice, electrical stimulation,
and ultrasound to improve your condition. All treatment is provided by highly skilled licensed therapists.
What insurance companies do you accept?
TMC Rehab accepts all insurances.
My physician recommended another place for rehabillitation but I would like
to come to TMC Rehab, what do I do?
As a patient you have a right to choose a physical therapy clinic that works for you and we recommend that you
discuss all of your treatment options openly with your referring physician.
Isn't physical therapy only for rehabilitation after surgery or injury?
Not at all. This is one of the greatest misconceptions about physical therapy.
Physical therapists work in many areas involving overall health and wellness --
such as injury prevention for workers at their jobsites, athletes preparing for competition,
